You will be based at Gravesham Community Hospital, Gravesham, as part of our community nursing team who provide care to people in their own homes or within a clinic setting.
We empower our staff to work autonomously and flexibly by providing state-of-the-art tablets, enabling them to check in from home, write notes on the go, and avoid being confined to an office, fostering a true community work environment.
As this is a community-based role, you will need a UK driving licence and access to a car.
Main duties of the job
Within this role, you will:
* Assist in providing nursing care as delegated by the Senior Sister/Team Sister within patients' homes.
* Maintain knowledge, skills, and attitudes necessary to deliver a quality, responsive service that adapts to patients' changing needs.
* Provide clerical and administrative support to the team as delegated by the Senior Sister/Team Sister.
* Undertake technical aspects of nursing care, such as dressings and glucose monitoring, following appropriate training and competence assessment.
* Implement evidence-based nursing care within patients' homes.
* Work independently, planning and organising tasks and home visits as delegated by the nurse in charge.
* Maintain accurate and legible patient records in line with professional and organisational standards, including electronic (CIS) and paper records.

Job responsibilities
This role offers an opportunity to utilize your NVQ Level 2/3 or equivalent qualification/experience. Strong communication, organisation skills, and a flexible approach will help you deliver positive patient experiences throughout their healthcare journey.
You must be able to travel across the area promptly, so a UK driving licence and access to a car are required. Confidence in IT systems and working knowledge of Microsoft Office (Outlook, Word, Excel) are also necessary.
