We are currently looking for a Care Home Liaison Officer to work as part of a busy team, based at Barr Lane to support the Hospital Social Work Team to support and facilitate safe and timely hospital discharges to care home settings across the Wakefield district and extended boundaries. As a Care Home Liaison Officer, you'll be the key link between Wakefield Council's Adult Social Care team and our partners in residential care. Your role will be vital in:
Responsibilities
* Effectively negotiate care home placements between two care settings with care home professionals to ensure service user needs continue to be met and ensure all requirements are in place to support safe transfer of care.
* To work in collaboration with assessment and care management colleagues to ensure safe transfer of care of medically fit patients from hospital to other settings.
* To provide first point of contact for health, social and care home professionals with regards to care home discharge updates either verbally or in writing, working within the Council's Customer Care policy and displaying a can-do attitude.
* To provide daily performance intelligence to Service Manager and Hospital Team Managers on a timely basis to support medically optimised for discharge intelligence received from the Acute NHS Trust.
* To develop, implement and review new systems, processes and more efficient and effective ways of working and to contribute to the implementation of new technology.
Qualifications
* Exceptional communication and relationship-building skills
* A proactive approach – identifying challenges and delivering effective solutions
* Strong organisational skills for managing visits, meetings, and follow-ups
* Knowledge of adult social care (or the willingness to learn quickly)
* Competent IT skills – maintaining accurate records and preparing insightful reports
