Overview

The Cambridgeshire Learning Disability Partnership (LDP) provides an integrated health and social care service for adults with Learning Disabilities living in Cambridgeshire. The LDP aims to ensure the assessment and provision of person-centred specialist health and social care services that promote an individual’s right to choice, independence, health and wellbeing, in line with their assessed needs and enables them to reach their full potential.

We have an exciting opportunity to recruit 3 enthusiastic Band 5 Community Learning Disability Nurses to work across Cambridgeshire. The posts are based in Cambridge South - Papworth, Fenland Team - March, Cambridgeshire and the East team are based in Ely.

You will have experience relevant to working within a community-based role, supporting people with learning disabilities. As an autonomous practitioner you will manage a caseload providing assessment and interventions in relation to the health needs of adults with a learning disability.


Responsibilities

* Participate in the delivery of specialist Learning Disability nursing service under the guidance of a senior Nurse.
* With support from a senior Nurse work with identified service users providing assessment, identification and treatment of the health needs of adults with a learning disability.
* Prioritise and maintain a caseload with the support of the senior nurse.
* Participate in the assessment, identification, treatment and management of the specialist health and nursing needs of the learning disability population, including risk assessment and management of complex conditions such as challenging behaviour, forensic issues, palliative care, mental health, epilepsy, etc.
* Provide advice and support to service users, carers and partner agencies regarding people with learning disabilities.
* Liaise and communicate effectively with other professionals in the MDT, the LDP and external agencies, providing specialist LD advice as necessary.
* Provide interventions in relation to the implementation and evaluation of specialist programmes of care for individuals or groups with learning disabilities and complex needs.
* Understand the specific needs of the service user group, including times of crisis (e.g., placement breakdowns, mental or physical health relapses, challenging behaviours).
* Recognise the impact of a person’s communication needs and tailor interactions accordingly.
* Demonstrate knowledge of multidisciplinary working with parents with learning disability.
* Work in service users’ homes and a variety of community settings, including lone working where appropriate.
* Under the supervision of the senior nurse, write timely professional reports/records and action plans in line with professional guidelines and local record-keeping policies.
* Provide nursing advice in relation to individual casework to professionals within the LDP.
* Work to reduce health inequalities for people with a Learning Disability by collaborating with partners in primary and secondary care.
* Understand local health needs and respond to changes in care provision as outlined in government initiatives.


Requirements / Qualifications

* Experience relevant to working within a community-based role, supporting people with learning disabilities.
* Ability to work as an autonomous practitioner and manage a caseload.
* Knowledge and understanding of working with adults with a learning disability and willingness to undertake appropriate post-registration training.
* Understanding of health and social care policy and legislation related to the service user group.
* Full UK driving license and use of a vehicle to travel independently around the county (note: applicants should confirm they meet this criterion in their application).
* Ability to work in a variety of settings, including service users’ homes and community settings; lone working may be required.
