Join an innovative community service where you can truly make a difference. We are excited to offer a Specialist Physiotherapy opportunity within our Long-Term Conditions Rehabilitation Service, designed to transform patient care and outcomes.
Our service brings together Specialist Falls, Cardiac Rehabilitation, and Moving Forward (previously Long Covid) teams to deliver holistic, patient‑centred care. Working collaboratively, we support individuals with a wide range of long‑term conditions to improve their quality of life.
In this role you will focus on empowering patients through effective symptom management, helping them take control of their health, wellbeing, and recovery. Using a variety of therapeutic approaches and self‑management strategies, you will play a key role in enabling lasting, meaningful change.
You will be involved in shaping and developing the service, delivering one‑to‑one and group interventions, and working within a forward‑thinking multidisciplinary team committed to high‑quality, evidence‑based practice.
Main duties of the job
* Provide highly specialist assessment, diagnosis, and treatment of patients with complex therapy needs as an autonomous practitioner within our symptom‑led Long‑Term Conditions Rehabilitation team.
* Manage a defined caseload using evidence‑based, patient‑centred principles to assess, plan, implement, evaluate, and record all interventions.
* Deliver virtual and face‑to‑face groups that educate patients about living with a long‑term condition and treating symptoms and associated needs.
* Provide one‑to‑one clinic sessions where patients require a more intensive approach to care.
* Work as part of an MDT and use an evidence‑based approach to managing patients with a range of conditions using a symptom‑led approach.
* Provide clinical education and training to others, including therapy colleagues and students.
Qualifications
* Degree in Physiotherapy
* Current registration with the appropriate governing body
* Evidence of relevant recent postgraduate experience and CPD
* Demonstrated postgraduate experience in a broad range of core areas, e.g. group delivery, long‑term conditions, rehabilitation, chronic pain, vocational rehabilitation
Experience
* Experience working with adults with long‑term conditions
* Experience of multidisciplinary team working
* Experience working in a community setting
* Experience working with patients with cardiac and respiratory symptoms, anxiety and depression
* Experience of audit, research, and service review
* Experience working with patients in groups or classes
* Contributions to the clinical education of students and MDT staff
Knowledge
* Knowledge of quality issues, governance, and outcome measures
* Understanding of the legal responsibilities of professional practice
Skills/Abilities
* Comprehend and work within the Trust's policies and data protection requirements
* Organise own workload and support junior staff and Therapy Assistants in their organisation
* Keep accurate and legible patient notes that meet legal requirements
* Prioritisation skills and ability to delegate effectively
* Able to undertake therapeutic handling, requiring moderate physical effort over a sustained period throughout the working day and to carry out concurrent activities
* Good communication and interpersonal skills
* Problem‑solving approach
* Passionate about service development and learning new skills
