Vulnerable people facing complex life challenges need consistent and seamless support to ensure they remain engaged with vital health and wellbeing services – and to prevent them becoming disenfranchised and isolated. Our combined expertise in the fields of substance use, mental health, housing, criminal justice, and employment support enables Adferiad Recovery to meet the needs of our most vulnerable with a single, unified, and comprehensive approach. Hours: 30 per week Salary: £25,809.30 per annum pro rata Based at: Cardiff Role Purpose: T he post-holder will be responsible for the day-to-day delivery of the Social Navigation model across the Cardiff and Vale University Health Board footprint and to liaise with strategic partners and others. The overall purpose of the Social Navigation Service is: To provide holistic, person centred support for adults experiencing social crisis presenting to the Mental Health Services within CVUHB. Utilising the Adferiad Recovery Programme to support individuals to address their immediate social crisis and enabling them to retain the tools they need to self-manage such situations in the future. Reducing ongoing and developing dependencies upon existing mental health services, whilst promoting the crisis response teams in CVUHB ability to address the mental health concerns of their caseloads. Assisting clients in identifying the key areas in which they require support e.g. accommodation, social support, physical health, finance, and money. Identifying suitable remedies to address identified issues, including liaising with local services both statutory and voluntary as appropriate to ensure the best outcome for the individual. To develop a suitable directory of services to address the needs of those affected by social crisis within the local communities of Cardiff and The Vale areas. Supporting vulnerable individuals to access appropriate services through the facilitation of an introduction wherever possible, including preliminary support work with the client to build their confidence and self-esteem.
