Please note, at this stage, the role is open only to internal staff (including NHSP and Agency staff currently on placement within our organisation) and staff substantively employed within Kent and Medway NHS Trusts and the Kent and Medway Integrated Care Board (ICB). Any individual who is at risk of redundancy will be given priority, provided they meet the essential criteria.
About the Service
The Community Rehabilitation Service provides a flexible range of mental health interventions to help clients with complex mental health needs manage their mental health more effectively and remain in their own homes. The team works in close partnership with Social Care and housing providers to ensure each client has the necessary resources to enable them to engage in their rehab journey. Across the Community Rehabilitation teams, the clinical team consists of Occupational Therapists, Psychologists, Nurses, and VCSE Workers.
Role Overview
We currently have exciting opportunities for two enthusiastic and motivated Healthcare Support Workers to join our East Kent Community Rehabilitation Service based in Folkestone but covering the East Kent area as required.
Responsibilities
* Support the team to provide input between the hours of 08:00 and 20:00, including some evenings, weekends and public holidays.
* Work as part of a Community Rehabilitation Team, focusing on the direct needs of service users.
* Under the guidance and supervision of the Multi‑Disciplinary Team, provide support and give time to an allocated group of service users to promote their recovery and maintain them in their community environment.
* Promote and facilitate therapeutic activity, self‑care and support with activities of daily living.
* Support service users to identify their needs and assist clinicians to plan, implement and evaluate individual care plans.
* Work within the recovery model to support the service user to work towards their identified goals.
* Give physical health checks and physical health care as required or instructed, document the checks on the early warning scoring system (MEWS), chart, and report changes to the nurse/healthcare professional in charge.
* Undertake day‑to‑day tasks as allocated by the nurse/healthcare professional.
Qualifications and Requirements
* Must hold a full, valid, UK driving licence.
* Have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
