Overview
The Community Children\'s nursing team is part of an integrated network of services for children in Bexley and Greenwich providing high quality specialist child centred care. The team provides highly skilled nursing care to children and families with nursing needs in their own homes or community settings. To provide a high standard of evidenced based nursing care for children and young people with acute and long term conditions, children with disabilities and complex conditions including those requiring palliative and end of life care. To provide clear visible leadership and professional clinical expertise to the Band 5 Children\'s Community Nurses in the whole team to ensure every aspect of the child and families experience is delivered in a safe, efficient and evidence based manner.
Responsibilities
* Lead and support Band 5 Children\'s Community Nurses with clinical expertise to ensure safe, evidence-based care.
* Hold a caseload of children with varied health conditions including complex medical needs, long-term conditions and short-term referrals under the Home-based care remit.
* Support more junior staff to develop skills and confidence in the CCN role.
* Work closely with Band 7 specialist nurses who will support learning and development for Band 6 nurses.
* Work at weekends and bank holidays on a rotational rota and support 24-hour end of life care as required.
Qualifications and requirements
* Experienced children\'s nurse with leadership skills and experience in either community or an acute setting.
* Interest in or willingness to develop and extend specialised skills and knowledge.
