Overview
We are seeking to recruit a motivated and committed Family Health Worker (FHW) to work within the Children and Young People’s Nurse (CYPN) Team within Children\'s Health. This role will focus on children aged 6-19 years (25 SEND) to support the CYPN’s with completing individual need work to support children\'s health and development. The CYPN FHW vacancy is based within Boston and South Holland. Due to recent changes in immigration law in the UK we are unable to offer skilled worker visa sponsorship for this role as it does not meet the requirements of RQF Level 6.
Responsibilities
* Deliver a package of care under the supervision of a CYPN, supporting areas such as sleep, toileting and behaviour to aid children\'s health and development.
* Work across a variety of settings (clinics, schools and homes) with flexibility to adapt to different environments and families.
* Drive across the locality/county as required to meet service need (the role supports across Lincolnshire, with expectation to travel).
* Provide group presentations to parents in support of health promotion activities (where appropriate).
* Maintain accurate electronic records and demonstrate excellent organisational and communication skills, both verbal and written.
* Share safeguarding concerns with your line manager and work collaboratively with Early Help and Social Care colleagues to improve outcomes for children and young people, under supervision of the CYPN.
* Maintain a positive \'can do\' attitude and be passionate about making a difference for the children and young people of Lincolnshire.
What we are looking for
* An understanding of child development and parent education initiatives.
* An understanding of children\'s safeguarding with a non-judgemental approach to supporting families and the ability to share and report concerns to your line manager.
* Proactive and capable of delivering a package of care under supervision of a CYPN.
* Experience and confidence to deliver group presentations to parents in support of health promotion activities (welcomed).
* Excellent organisational and communication skills, both verbal and written. Good IT skills essential for electronic record keeping.
* Ability to work in several different environments and adapt to the needs of the family and child.
* Commitment to working collaboratively and in partnership with Early Help and Social Care colleagues to improve outcomes under the supervision of the CYPN.
* Positive \'can do\' attitude and passion for making a difference for the children and young people of Lincolnshire.
