Are you a passionate and experienced leader ready to make a real difference in adult social care? We have an exciting opportunity for a Service Manager to lead our newly established Adult Social Care Hub in Bath and North East Somerset.
About the Role:
As Service Manager, you will oversee a dynamic hub that supports all new referrals into adult social care. You will manage several key teams, including:
- Hospital Social Work Team: Focused on discharging individuals from acute and community hospital settings and conducting Care Act assessments.
- Community Pathways Team: Ensuring timely assessments and smooth transitions for individuals receiving reablement or intermediate care.
- Safeguarding Triage and First Response Team: Providing urgent support and safeguarding interventions.
- Access Advice and Information Team: Delivering person-centred advice, information, and signposting to help people achieve their desired outcomes.
Your leadership will ensure seamless coordination across these teams to provide high-quality, timely support that meets the long-term care and support needs of our community.
What We’re Looking For:
- Proven experience managing adult social care services or similar health and social care teams.
- Strong understanding of Care Act assessments, safeguarding, and discharge-to-assess pathways.
- Excellent leadership and communication skills with a person-centred approach.
- Ability to work collaboratively with multi-disciplinary teams and community partners.
- Commitment to delivering high-quality, timely care and support.
