<div><h3>Practice Nurse (Maternity Cover, Fixed Term)</h3><p>We are seeking an enthusiastic, experienced, and highly motivated Full or Part Time <b>Practice Nurse</b> to join our supportive nursing team on a <b>fixed-term basis</b> to cover maternity leave. The role is ideal for someone who is passionate about patient care and proactive in improving quality within a Primary Care setting.</p><h3>Main duties of the job</h3><p>The successful applicant must:</p><ul><li>Hold more than one academic Level 6 diploma or postgraduate certification in long-term conditions care and/or public health initiatives.</li><li>Demonstrable experience in leading long-term conditions management and quality improvement.</li><li>Is registered with the NMC and maintains revalidation in line with NMC requirements; and is working at Registered Nurse Level Practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework.</li><li>Provide and maintain our high standards of nursing care.</li><li>Duties will include all tasks normally undertaken by an experienced Practice Nurse and any additional roles agreed between the nurse and the doctors as appropriate, having regard to current training.</li></ul><h3>About us</h3><p>We are a dynamic Training practice with over 11,000 patients, boasting an expanding multidisciplinary team, incredibly supportive nursing, management and administrative team and based in recently refurbished premises in Sheffield Centre.</p><p>Consistently voted in the top of Sheffields GP practices for patient care. We continue to develop our ways of work to best suit our patients, staff and the ever-changing needs of the world we live in.</p><p>We are an active member of our community and PCN with a strong Training history, Care Home record and are involved in the development of shared clinical roles.</p><p>TPP SystmOne clinical system</p><p>High QOF achievers</p><h3>Job responsibilities</h3><p>Key Responsibilities You will undertake a broad range of duties including but not limited to:</p><ul><li>Providing high-quality, evidence-based nursing care in line with clinical protocols.</li><li>Assessment, planning, implementation, and evaluation of care for patients with, or at risk of, long-term conditions.</li><li>Leading quality improvement initiatives and managing patients at risk of developing chronic illnesses.</li><li>Administering childhood and adult immunisations, travel vaccinations, and cervical screening.</li><li>Delivering health promotion and lifestyle advice on areas such as smoking cessation, diet, and physical activity.</li><li>Performing wound care, suture removal, and management of minor illnesses within scope.</li><li>Providing contraceptive services, including Depo-Provera injections, and general sexual health advice.</li><li>Supporting infection prevention and control (IPC) activities including audits and training.</li><li>Maintaining accurate and confidential clinical records in line with legislation and best practice.</li><li>Attending relevant training sessions and keeping up to date with CPD and professional development.</li></ul><h3>Person Specification</h3><h3>Key Attributes</h3><ul><li>Compassionate, patient-centred approach.</li><li>Excellent communication and interpersonal skills.</li><li>Ability to work independently and as part of a team.</li><li>Strong organisational and time management skills.</li><li>Commitment to continuous professional development and clinical</li></ul><h3>Experience</h3><ul><li>Experience in a Primary Care setting.</li><li>Demonstrable experience in chronic disease management (e.g., diabetes, asthma, COPD).</li><li>Experience working within defined clinical protocols and as part of a multi-disciplinary team.</li><li>Previous experience working autonomously.</li><li>Experience as a Practice Nurse or Community Nurse.</li><li>Working with Learning Disability, elderly or housebound patients.</li><li>Involvement in quality improvement initiatives.</li><li>Willingness to undertake further training and development.</li></ul><h3>Qualifications</h3><ul><li>NMC Registered Nurse (with active registration and compliance with revalidation).</li><li>More than one Level 6 diploma or postgraduate qualification in long-term condition care and/or public health initiatives.</li></ul><h3>Disclosure and Barring Service Check</h3><p>This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.</p></div>
