As one of our Care Co-ordinators, you will be responsible forsupporting the practice in delivering excellent patient care by effectivelyco-ordinating different elements of care from multi-disciplinary teams withinand outside the practice.
We are currently seeking to recruit an additional CareCo-ordinator to join our Proactive Care Team.Working alongside our existing Care Coordinator team, Social PrescribingLink Workers and Health & Wellbeing Coaches as well as our GPs and clinicalteams, you will coordinate care suited to the needs of individual patients.

Main duties of the job
Our successful candidate will have a busy and varied roleincluding service co-ordination, care planning for patients and providingpatient support. You will co-ordinatemultidisciplinary team meetings, liaise with clinical and non-clinical teamswithin Holderness Health and across our partner agencies.
Acting as a key point of contact for patients, carers andhealthcare professionals, you will ensure that an individuals healthcare needsare addressed in a joined-up way, ensuring that they receive the right carefrom the right professional at the right time.
You will have excellent communication and negotiation skills andbe confident managing your own workload.Our ideal candidate will have experience of working in a health orsocial care setting and a deep understanding of patient care needs but we arealso interested in hearing from people from a wide range of backgrounds who candemonstrate sound administrative skills and experience of delivering highquality customer or patient care.
Contact with patients is predominantly via telephone, thereforeyou must possess an excellent telephone manner.

We are a large rural practice with approximately 34,000patients. As a single-practice PrimaryCare Network, we have a wonderful opportunity to transform care for ourpatients. We operate from 7 locationsacross Holderness and you must also be willing to travel between sites asnecessary. However the role will be predominantly based on Hedon Road, Hull.

Role Purpose:

ServiceCo-ordination
Provide coordination and navigation for patientsand their carers across health and care services, working closely with Social PrescribingLink Workers, Health and Wellbeing Coaches, and other primary careprofessionals
Co-ordinate the work of healthcare professionalsand non-clinical staff involved in thecare of patients registered at the practice
Co-ordinate MDT meetings, collating informationon patients requiring review and providing secretarial and administrativesupport, and ensuring the completion of resulting actions
Act on communications from hospitals, communityservices, mental health services and other providers
Facilitate inter-agency communication to supportthe discharge and handover of patients between different health and caresettings
Support Lead GPs with the administrative aspectsof QOF (Quality Outcomes Framework)
Act as a single point of contact for healthprofessionals and patients within your remit
Active CarePlanning
Work with GPs and other primary careprofessionals to identify and manage patients
Work with defined cohorts of patients, focusingon what matters to the person
Help people to manage their health needs throughanswering queries, making appointments, booking tests and arranging otherreviews including long-term condition reviews
Ensuring that actions from review meetings areprogressed on behalf of patients
