Remuneration: Circa £120,000 Take Home for full time
Description: Part Time/Full Time Performer Dentist required 3-5 days per week to work for a busy practice in Winterbourne, near Bristol.
Key Details:
1. Days and Hours Flexible
2. £14 per UDA
3. Up to 7000 UDAs
4. Ideally an NHS Mentor (£500 monthly mentor payment)
5. Hours 8.30am - 5pm (need to be available for 10am - 7pm on a Tuesday, if required) Also open 9am - 1pm on a Saturday
6. 8 Computerised surgeries
7. Equipment: Digital x-rays, Rotary Endo & Apex Locator, iTero Scanner, OPG, CBCT, Microscope, Intra Oral scanner

Compliance Requirements:
1. Full NHS Performer Number
2. Local Area Team confirmation
3. GDC certificate
4. UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years
5. Indemnity certificate
6. Passport
7. Enhanced DBS (within the last 12-36 months)
8. Clinical references covering places of employment over past 2 years (minimum 2 referees)
9. Degree certificate
10. CPR/AED/Medical Emergencies certificate dated within the last 12 months
