 
        
        The role involves working directlywith the practices and the multidisciplinary team within the PCN. The role ispivotal in ensuring all patients receive the best possible care and servicesfrom the GP Practices members.
The Care Coordinators role willsupport the member practices in coordinating all key activities includingannual reviews, access to services, advice, and information, and ensuringhealth and care planning is timely, efficient, and patient-centered.
The role will include supportingdigital initiatives such as increasing the uptake and use of the NHS App and includes responsibilities for the co-ordination of thepatients journey through primary care.
Main duties of the job
1.Works within the primary careteam, contributing to leadership of service evaluation and research to promotequality improvement activity.
2.Works with the PracticesPatient Participation Group to support communication with the wider patientcommunity.
3.Collaborates with other membersof the team, patients and their carers when managing and coordinating care.
4.Uses healthcare technologies tooptimise service delivery, patients access, and continuity of care

We are in the very fortunate position where 4 of the PrimaryCare Networks (PCNs) in Redbridge are working together as part of theRedbridge-wide GP federation Healthbridge Direct.
This means we can build strong clinical teams within andacross the PCNs. We are now at the very start of an exciting journey ofbuilding a team of over 100 additional clinical roles to support and enable thedelivery of services within each of our 4 PCNs. We are building teams withinand across the PCNs to enable peer support and team working both within andacross professions.

Job responsibilities
The Care Coordinatorresponsibilities include but are not limited to the following:
1.The focus of the role will be to provide admin supportdirectly to GP practices within Marshalls PCN.
2.To work with the GPs and other primary careprofessionals within the practice to provide support for the call and recall ofpatients on long term condition registers for the Quality and Outcome Frameworksand Local Enhanced Services and PCN Directed Enhanced Services ensuring targetsare met.
3.To support the practice in promoting Public HealthInitiatives and Campaigns.
4.To actively support the practices activities aroundimmunisations including the delivery of childhood immunisations, influenza andcovid vaccination programs.
5.To support practices in the identification ofcarers. To signpost and provide support and information.
6.To support practices in contacting thehard-to-reach patients and those that do not respond to communications.
7.To support patients to utilise decision aids, helpcreate single personalised care and support plans, in line with best practice.
8.To help people to manage their needs throughanswering queries, making, and managing appointments
9.To provide coordination and navigation with the aidof digital tools for people and their carers across health and care services.Including increasing the uptake and use of the NHS App and other applicationsto promote the practice and the additional roles and services being providedfrom practices.
10. To supportthe coordination and delivery of MDTs within the PCN.
Person Specification
Experience
 * Must have some experience of working in general practice.
 * Must have experience of either working in social care or acute community care
 * Experience or understanding of co-ordinating an MDT in a health or social care setting
 * Excellent organisational skills
