Take a lead with case/care planning management for individual children or young people, undertaking assessment of need and risk, developing a care plan, and identifying agreed outcomes. Carry out regular reviews in conjunction with young people and other professionals.
Lead and work on shifts with accountability for service delivery, assessing and managing risks, deploying staff and resources accordingly while on shift, and maintaining personalized, high-quality, outcome-focused care for young people. Apply organizational procedures to ensure the safe care and management of children and young people in our service.
Supervise, guide, or direct staff and volunteers as required, providing mentoring to new staff, utilizing Action for Children policies and procedures.
Support children and young people in their personal care (where applicable), administration of medication, and clinical procedures (where applicable) in line with organizational policies and regulatory requirements.
Build and maintain professional, positive working relationships by providing emotional and social support to children and young people who may display challenging behaviors.
Work within the community to assist the young person and their family, promoting communication and participation by ensuring their views and needs are articulated, heard, and responded to.
Consistently apply organizational policies, procedures, and professional standards in line with registration requirements.
Ensure all records, both paper and electronic, are maintained and uploaded into the appropriate database.
Undertake assessments of young people, coordinating multi-agency and family information, analyzing and producing reports. Attend professional meetings, including reviews and court work, and refer cases requiring specialist support or management oversight.
Develop effective working relationships with team members, partner agencies, other external agencies, and colleagues within Action for Children.
