This is a 1 year Fixed-Term Maternity Leave Cover 10PA post (8 DCC, 2 SPA) based at St Mary's Hospital Campus of Imperial College Healthcare NHS Trust.

Medicine for the Elderly (MfE) services have evolved over recent years at St Mary's Hospital so there is now a multiprofessional Acute Frailty Team and ED liaison service, in addition to the 48 acute MFE beds across 3 wards, outpatients and liaison work with multiple community services in West London. The acute frailty team requires daily consultant input and this post allows for year‑round provision of acute frailty input on admissions wards and into the Emergency Department, and further to allow the development of Frailty OPRAC at the SMH site.

Contract Duration: 12 months


Main duties of the job

Frailty service development is at the heart of the Trust's planning and operations, and the addition of this post holder will allow not only provide the requisite level of cover to maintain a constant service, but also the capacity to design and pilot improvements within the team. There is an active programme of audit and QUIP within and around the department and an active working group with the ED who have an appointed frailty lead. The post holder will be able to participate on local and regional working groups and CQUINs - we are currently signed up to the Frailty Assessment CQUIN in ED.

This post holder will work with 1 consultant colleagues in the frailty service and will be primarily based in the Emergency Department and Acute Medical Unit, providing acute frailty input to admitted medical patients. Additionally, supervision and collaboration with the emergency department‑based Frailty Practitioners (ACPs) is a key component. There will also be opportunities for involvement in community and virtual ward input, and general medicine admissions / on‑call shifts. Job responsibilities

* Inpatient beds on all sites for frail, complex older patients requiring hospital care
* A rehabilitation unit at the CXH site
* Acute Frailty teams on both acute sites providing comprehensive geriatric assessment to all complex older patients admitted on the acute medical take
* Dedicated Surgical liaison service orthopaedics, vascular and GI surgery
* Dedicated Cardiology and Renal liaison services
* Community input to local care homes, Primary Care Network Frailty hubs, Virtual Wards and the Community Independence Service (CIS), an integrated health and social care provision to support older patients in the community

All services and care are based on the Comprehensive Geriatric Assessment and effective multidisciplinary working. They are aligned with the evidence base and national guidelines.

The Department of Elderly Medicine at St Marys Hospital is currently staffed by 4 Consultants:

* Dr Colin Mitchell (Falls, Divisional ICT lead)
* Dr David James (Dementia lead)
* Dr Susy Long (Chief Medical Examiner, Patient Safety Research)
* Dr Alvin Shrestha (Acute Frailty)


Person Specification

* On GMC's register with a Licence to Practice or eligible to apply for GMC registration and a Licence to Practice
* On GMC's Specialist Register or within six months of being admitted to the register by the date of the interviews or eligible for specialist registration
* MRCP


Knowledge/ Skills

* Extensive experience in Elderly Medicine
* Excellent clinician with thorough grounding in Elderly Medicine
* IT Skills and computer literacy


Leadership/ Management

* Able to take responsibility, show leadership, make decisions, exert appropriate authority
* Have proven skills in leading, motivating, developing and managing the performance of colleagues.
* Knowledge of finance/budgets Management of staff


Teaching & Training

* Experience of teaching and training undergraduates/postgraduates and junior medical staff


Audit

* Understanding of principles of clinical audit


Academic achievements including research/publications

* Evidence of achievement appropriate to appointment at consultant level at Imperial College Healthcare NHS Trust


Language

* Are able to speak and write English to the appropriate standard necessary to fulfil the job requirements


Probity

* Honesty, integrity, appreciation of ethical dilemmas.
* Must be able to demonstrate and model the key Trust values of kind, expert, collaborative and aspirational

Depending on experience £109,725 to £145,478 per annum WTE
