The community Rapid Response team at Blackpool Teaching hospitals is delighted to be recruiting a non medical consultant to clinically lead the service and develop it into the future. This is a newly created post and the post holder will be a clinical expert in acute community care and will fulfil the 4 domains of the non- medical consultant.
1. Advanced and expert level practice within the field of Long-Term Conditions management in community care.

The successful candidate will work with the integrated team of nurses, therapists, and social care, to provide holistic care for our patients. You will lead the Rapid Response Service to develop pathways and provide clinical oversight and education for the team. The rapid response team provide interventions to prevent hospital admissions and facilitate discharges, meet 2 hour Urgent Community Response standard guidance, and support patients on the Frailty and Respiratory Virtual ward.
Main duties of the job
We have a well-established integrated service and are now seeking to add further clinical leadership in the newly formed post of non - medical consultant.
Crisis may be due to a clinical condition which is a new or acute problem such as infection, an exacerbation of a chronic condition or a social crisis such as breakdown of unpaid carer arrangements causing an immediate health risk.
The Rapid Response Team provides an alternative pathway that will treat people at home where it is safe to do so to reduce unnecessary hospital admissions and facilitate early safe discharge.
You will be able to carry out comprehensive assessments and make autonomous decisions based on the data obtained. You will be a key decision maker on the patient's pathway during and post crisis.

Job responsibilities
The post holder will practice at an advanced clinical level and exercise high levels of judgement, discretion and decision making in clinical care. They will provide clinical leadership, expert practice and advanced knowledge, integrating research evidence into practice and contributing to/leading on research in their clinical field.
There is an expectation that a minimum of 50% of the post holders portfolio will be spent in direct autonomous clinical practice having clinical responsibility for a defined cohort of patients and providing leadership for clinicians within this clinical area.
The post holder will monitor and improve standards of care through supervision of practice, clinical audit, identifying research questions, disseminating research findings and promoting their translation into practice, teaching and supporting professional colleagues and the provision of skilled professional leadership.
The post holder will contribute to safeguarding and clinical effectiveness within the service by participating in audit and research as part of the MDT within the Rapid response service across the Fylde Coast.
As a member of the MDT, the post holder will also be involved with, and where appropriate collaborate in, national and international research contributing to relevant networks and forums, with the aim of supporting the development of a centre of excellence in care and treatment for community patients.

Essential
* Registered Nurse on NMC register or registered therapist on HCPC register
* MSc in advanced practice or successful NHSE credentialling with E- portfolio

Experiance

* Experience of clinically leading a service
* Experience of manging patients with chronic and acute presentations who have long term conditions
* Significant evidence of delivering teaching Programmes to staff groups
* Previous experience of working autonomously as a Senior Nurse/AHP (Band 8a) within the speciality.
* Experience of holding clinical responsibility for a whole patient pathway
* Experience of planning, assessing and directing care provided to patients across a whole pathway.
* Experience of service development and redesign
* Significant experience of training and developing staff
* The post holder must have regular access to transport to meet the demands of the role
* Evidence of leading Audit / research
