Central Bournemouth Primary Care Network is dedicated to providing high-quality primary care services to the local community. With four GP surgeries and six sites, we serve approximately 50,000 patients in the Moordown, Winton, Charminster, and Muscliffe areas.
Summary
We are seeking a Care Coordinator to work primarily with our Assessment and Care Team (AC Team) but also to support the Self-Management Team.
Hours and Base
Based at our East Way Clinic, BH8 9PU. This is a part time post with 30 hours per week available.
Responsibilities
* Co-ordinate weekly Care Home "ward round" including contacting homes on Mondays for patient lists, noting outcomes of Tuesday MDT meeting and updating patient records.
* Maintaining patient lists of residents in Care Homes and monitoring when new Dorset Care Plans are required.
* Supporting seasonal vaccination programmes including stock supplies, consent records, attending Care Homes with clinicians and updating clinical notes.
* Managing all incoming tasks for the AC Team on SystmOne. Booking visits with appropriate clinicians, onward escalation where appropriate and updating practices with treatment plans.
* Ordering stationery and medical equipment supplies.
* Running Self-Management triage meeting on Mondays and updating patient records with discussion outcomes.
* Monitoring incoming referrals for Self-Management Team and allocated to most appropriate member of staff or service.
* Acting as PCN Lead for Period Poverty. Maintaining supplies and liaising with practice contacts to ensure other sites are well stocked. Working with community providers to receive donations of supplies.
* Quality checking voluntary sector organisations, statutory services and grassroots community groups. This may include site visits. Recording up to date information about them on PCN Sharepoint.
* Upkeep of peer support group attendance records.
Qualifications
* Proven experience in an office environment or healthcare setting.
* GCSE/O Levels
* Strong organizational skills with attention to detail.
* Excellent communication and interpersonal abilities including fluent English.
* Ability to work collaboratively within a team-oriented environment.
* Proficiency in using office software and managing electronic records.
* Knowledge of healthcare systems is a plus.

Pay: £12.86-£15.12 per hour
