Overview
As a Team Leader at Avery Healthcare, you will be responsible for supporting the home management team in delivering high-quality, person-centered care. Your focus will be on creating a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff. Your role includes leading in the care planning process, assisting with resident discharges, monitoring resident well-being, and ensuring safe systems of work.
Job responsibilities
* Package Description: Shift Details - Night shift with alternate weekends.
* Lead in the person-centred care planning process by implementing, monitoring, undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans.
* Assist in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents\' next of kin fully informed, where appropriate.
* Lead with monitoring the well-being of each resident, being aware of any change impacting care and care planning and ensuring that changing needs are met and written records are amended to reflect any variation.
* Practice safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
About You

Our ideal candidate must:
* Have an NVQ 3 or equivalent in care for care homes.
* Have two years experience working in a senior position with the resident group.
* Have the ability to communicate effectively both verbally and in writing.
* Demonstrate the ability to lead staff and act responsibly in the absence of the manager.
