Overview
The Mindworks Hub Clinical Practitioner (Social Worker, Nurse or Occupational Therapist) will be a member of the multidisciplinary specialist team, providing services to children and young people 5‑18 years and young people in transition to adult services with mental health needs, emotional and behavioural problems. The post holder will provide assessments and treatment, individually or with a multidisciplinary team member, within both clinic and community settings, while offering a range of therapeutic interventions appropriate to the young person’s care pathway.
Responsibilities
* Provide assessments and treatment, individually or with the multidisciplinary team, within clinic and community settings.
* Offer a range of therapeutic interventions appropriate to the young person’s care pathway.
* Support the team in leading and embedding the THRIVE Framework across the newly created Mindworks CYPS in North Surrey.
* Carry own caseload and undertake holistic assessments, including risk for the child/adolescent/family, individually or in complex cases jointly with another CYPS Mindworks member.
* Provide short‑term treatment to CYP.
* Liaise with Social Care, Education and other appropriate agencies, professionals and carers to ensure best outcomes for CYP.
* Act as a lead professional for CYP following initial assessment and develop an appropriate plan of care.
* Offer support for students on placement.
* Co‑work with other members of the team (MDT).
* Maintain accurate and timely records of all work undertaken in accordance with Trust policies and professional body requirements.
* Provide statistical information on all activities undertaken, in line with Trust procedures, and use basic IT and data entry skills to enter clinical information into the database.
* Contribute to the team’s overall standard of services provided.
* Promote effective liaison with all statutory and voluntary agencies concerned with CYP and mental health.
* Use the appraisal and supervision systems to identify training needs and undertake continuous professional development.
* Ensure high standards of care, dignity, individuality, self‑esteem and wellbeing of individuals and their families.
