Mid Yorkshire Teaching NHS Trust is pleased to announce these innovative posts. The role is designed for doctors who have successfully completed their FY2 placements and wish to gain further experience in Adult Community Services, including the Hospital at Home. Working hours are Monday to Friday, 09:00 to 17:00.
Job responsibilities
* The postholder will support the Hospital at Home and admission avoidance pathways, contributing to the delivery of care within community settings including patients’ own homes.
* Exposure to alternatives to hospital admission and supported discharge models.
* Collaboration with community nurses, therapists, advanced care practitioners, primary care teams, acute hospital services, adult social care, and VCSE partners.

Essential criteria
* Full GMC Registration with licence to practise.
* Medical degree from a recognised medical school.
* Successful FY2 ARCP outcome or equivalent.
* IT experience (i.e., Microsoft Office packages).
* Eligibility to work in the UK.
* Completion of 1 year training within an NHS organisation.
* Recent minimum 6 months paid experience working in UK NHS hospitals.
* Demonstrates an intelligent and analytical approach to problem solving.
