Overview
Are you ready to be part of a new initiative at Salisbury NHS Trust? The Trust is investing in our Early Supportive Discharge (ESD) team to enable patients to return home as soon as medically fit, with additional care provided by the Council. The service operates a 'discharge to assess' model in partnership with Wiltshire Council's reablement team to provide therapy and support in the patient's own home.
Responsibilities
* Facilitate discharge plans and liaise with other services before taking patients home.
* Complete assessments in patients' own homes to ensure adequate care and therapy for a successful return to independence.
* Communicate effectively with patients, families and the wider team, supporting senior staff in planning and coordinating discharges.
* Organise and administer required documentation and collect data to evidence service outcomes.
* Provide care and practice exercise plans in patients' homes.
* Attend handovers, complete care referrals and support the patient’s therapy journey from admission through to discharge/community.
* Support patients with activities of daily living, including personal care and meal provision.
Working Hours and Patterns
The ESD team operates 7 days a week from 08:00 to 18:00. A standard working week is 08:00 to 16:00. Flexibility in working patterns is required.
Qualifications and Experience
* Full driving licence and access to a car.
* Right to work in the UK. Candidates requiring sponsorship will need to meet eligibility criteria on gov.uk.
* Rehabilitation or caring experience is desirable but not essential.
