Job Description: Community Nurse
The role of Community Nurse involves working in partnership with service users within a designated care group, particularly those with complex health presentations. Responsibilities include developing care and recovery plans to enable individuals to lead fulfilling lives. The nurse will demonstrate understanding of clinical interventions relevant to the care group, including education, care planning, and discharge planning, working closely with carers, families, and partner agencies. The post holder will always promote the safety and well-being of service users and their families/carers. Tasks include assessing, planning, and implementing care, providing specialist nursing advice, carrying out specialist procedures, supervising staff and students, and mentoring student nurses.
This position offers the Trust's £2,000 Golden Hello or Relocation Incentive for Band 5-6 Nurses & Practitioners (pro rata and subject to terms and conditions; see attached protocol for details).
Main Duties
* Manage a caseload of service users with complex health needs, undertaking assessments and developing care plans collaboratively.
* Provide specialist nursing assessments and advice as required.
* Assess and manage ongoing risks, reviewing and modifying risk management strategies in collaboration with service users and carers.
* Contribute to the development of the Trust's nursing strategy.
* Ensure clinical practice is evidence-based, aligned with NICE guidelines and Trust policies.
* Supervise junior staff and trainees appropriately.
* Adhere to the NMC code of conduct at all times.

Southdown Partnership Trust (SPFT) offers specialist mental health services to Sussex. The Service for Older Adults Mental Health Support (SOAMHS) provides mental health support to the older population in Hastings and Rother, aiming to help individuals remain in the community. Job Responsibilities
As a core team member, you will use specialist health assessments to deliver a range of clinical interventions relevant to the care group, with knowledge of evidence-based models of practice. For mental health posts, care will be delivered under the Care Programme Approach (CPA), with support from Assessment and Treatment Centres, acting as a care coordinator within CPA policies. For Learning Disabilities (LD) and Serious Mental Illness (SMI), caseload management will be central, working in partnership with individuals, families, and carers to ensure effective care plans that promote safety, well-being, independence, and community inclusion. This may involve working with partner agencies such as voluntary organizations, schools, and colleges to support service users in utilizing self-management tools. Care plans should be tailored to individual needs, strengths, and outcomes, with interventions designed to meet therapeutic goals. Continuous monitoring, evaluation, and modification of treatments will be necessary to measure progress and ensure effectiveness.
