Overview
We are seeking a dedicated and compassionate Care Coordinator to join our team. Duties
* Coordinate and oversee individual care plans in collaboration with healthcare professionals and clients
* Schedule appointments, follow-ups, and necessary services for clients
* Maintain accurate and up-to-date records of client interactions and care activities
* Communicate effectively with clients, families, and healthcare providers to ensure seamless care delivery
* Respond promptly to client enquiries and provide appropriate information or assistance
* Assist in the organisation of community resources and support services as needed
* Monitor client progress and report any concerns or changes to relevant parties
* Support team members with administrative tasks related to client care management
Qualifications
* Proven office experience demonstrating organisational and administrative skills
* Excellent communication skills, both written and verbal
* Compassionate approach with a strong commitment to client well-being
* Ability to work independently as well as part of a multidisciplinary team
* Organised with attention to detail and the ability to manage multiple priorities effectively
* Proficient in basic computer applications such as Microsoft Office (Word, Excel, Outlook)
* Previous experience in healthcare or social services is desirable but not essential
