Due to continued service growth and aged care reform, we are seeking an experienced and motivated Home Care Team Lead to join our Home and Community Care team. About the Role Reporting to the Executive Director Community Services, the Home Care Team Lead provides operational leadership across the Support at Home service. This role is responsible for overseeing service delivery, ensuring high-quality care outcomes, maintaining regulatory compliance, and supporting service development aligned with aged care reforms. You will lead a multidisciplinary team, supporting the delivery of safe, consumer-focused services while driving continuous improvement and workforce capability. Key Responsibilities Oversee day-to-day operations of the Support at Home service, including care planning, service coordination and individual client budget oversight Ensure services are delivered efficiently, equitably and in line with Support at Home program requirements and client goals Maintain compliance with the Aged Care Act, Aged Care Quality Standards and relevant funding and regulatory frameworks Provide leadership, supervision and performance management to Care Partners, Service Coordinators and support staff Oversee care management activities, including review of care plans, service effectiveness and management of complex or high-risk cases Support safeguarding, risk management and incident reporting processes Lead and support change initiatives associated with aged care reforms and the transition to Support at Home Monitor service utilisation, workforce capacity and client expenditure to support sustainable service delivery Collaborate with internal teams, external providers and key stakeholders to support integrated client care About You To be successful in this role, you will bring: Bachelor qualification in Nursing, Allied Health, Health Science, Social Work or related discipline Demonstrated leadership experience within community aged care services Strong knowledge of Support at Home program requirements and contemporary aged care frameworks Proven experience in care management, service planning and complex case coordination Strong understanding of aged care legislation, quality standards and compliance requirements Highly developed organisational, analytical and problem-solving skills Excellent communication and stakeholder engagement skills Ability to lead, coach and support multidisciplinary teams Current Victorian driver’s licence Benefits We offer a supportive and purpose-driven work environment, including: Salary packaging benefits – up to $15,900 tax-free plus $2,650 for meals and entertainment Flexible work arrangements to support work-life balance Opportunity to play a key leadership role during significant aged care reform Ongoing training and professional development opportunities Free on-site parking with convenient access to public transport and local amenities If you are passionate about leading high-quality community aged care services and making a meaningful difference in the lives of older people, we would love to hear from you. by submitting your resume and cover letter outlining your suitability for the role.
