Join to apply for the Activities and Well-Being Coordinator role at CCH Group. The Health and Wellbeing Coordinator plans and delivers engaging exercise and cognitive stimulation therapy sessions to improve participants\' physical and mental wellbeing and to motivate and inspire older people to maintain and improve their health and fitness. This role is accountable for the wellbeing of the service user/client and involves forming relationships with internal stakeholders and other professionals as needed.
Responsibilities
* Be an active member of the Care Team.
* Plan and deliver a range of group exercise classes for older people, including wellbeing initiatives such as exercises, wellbeing workshops, and signposting to wellbeing practitioners.
* Support a developing Dementia Project with the planning and coordination of a program of cognitive stimulation therapy sessions and peer support for people living with dementia.
* Design and adapt class content to suit participants\' abilities.
* Promote awareness around health improvement in older people.
* Provide support, motivation and encouragement to help clients achieve their goals.
* Maintain client records and monitor progress.
* Build relationships with clients and support them safely and effectively in all class sessions.
* Check fitness equipment for safety and correct use.
* Undertake necessary risk assessments.
* Ensure organisational policies and procedures are observed and implemented during service delivery.
* Provide first aid if required.
* Undertake any other duties as delegated by the Branch Manager.
