Hence, we require a community mental health nurse, who is able to work under pressure and enjoy

This role involves screening, assessing and providing medium to long-term intervention to local residents who require support from mental health services.
The role is structured around the neighbourhood model of care, which is about understanding the needs of local communities, building supportive
social networks and strengthening the resilience of service users. The service is made up of practitioners from different professional backgrounds
and we work closely with GPs, the voluntary sector and other teams within the mental health service to ensure that residents’ needs are met.
You need experience of working with adults with serious and enduring mental health issues using the strength-based model. You will need to have
good understanding of the principles of the Care Act, Mental Capacity Act and Mental Health Act and experience of safeguarding vulnerable adults.
Main duties of the job
To provide a high quality and effective mental health support and care to Hackney residents. To
respond to the needs of adults with mental disorder and their carers in a timely manner, within the
boundaries of statutory legislation, and with regard to individual circumstances. To assess need using a
person-centred approach and effectively assess and manage risk. To plan and deliver outcome focused
interventions linked to promoting independence & social inclusion. To act as an allocated worker taking
responsibility for the provision of community services to mental health services users and carers.

Detailed job description and main responsibilities
Experience of working in Adult Community Mental Health field is essential for this post.
As a member of the Recovery team, you will have the responsibility to exercise autonomous professional judgment in managing a clinical caseload
independently. This includes providing specialized assessment and follow-up care to individuals referred with moderate to severe mental
health problems in various settings.
Additionally, you will ensure that family and carers in a caring role receive
evidence-based assessments for support whilst caring. It will be essential for you to accurately and
contemporaneously document your work in the RiO system, recording and updating information as needed.
You will be accountable for your professional actions and expected to seek guidance and supervision from your line manager when facing ethical
dilemmas or in relation to Trust policies.

Education/ Qualification/ Training
Essential criteria
1. Evidence of professional qualification in nursing
2. Evidence of registration on Part 3 of NMC register
3. Evidence of continuing professional development including recently in perinatal mental health practice and safeguarding children practice
4. Educated to degree level or equivalent
Desirable criteria
5. ENB 998 or equivalent
6. Clinical supervision training
7. Qualified non-medical prescriber
8. Qualifications/ training in specialist skills e.g. therapeutic/ psychological interventions
Experience
Essential criteria
9. Minimum 2 years post qualification experience
10. Experience of multi-agency pre-birth planning (CPA) for women with serious E E S/I I Page 2 of 5 mental illness
11. Experience of managing a caseload of service users via CPA with evidence of completing high quality assessments
12. Experience of working in a complex multi-disciplinary/ multi-agency context
Desirable criteria
13. Experience of planning and delivery of training in response to identified needs
14. Experience of contributing to policies and procedures
15. Experience of supervising mental health professionals and/or practice teaching
16. Minimum 2 years post qualification experience working with women with serious mental illness
Knowledge and Skills
Essential criteria
17. Up to date knowledge of evidence based practice and current developments in perinatal mental health
18. Knowledge about mental illness in the perinatal period
19. Up to date knowledge of the Children Act 1989 and associated legislation and government policy and its implications for mental health services
20. Knowledge and understanding of diversity and equality issues and their application in perinatal mental health services
21. Knowledge & understanding of contributions of other disciplines and professionals to a multi-agency service
