Band 7 clinical lead - solihull integrated addiction service team

Responsibilities

* To be responsible as the SIAS Clinical Lead for the assessment and coordination of care needs, planning and implementation of evidence-based care for Solihull service users who meet the threshold of SIAS
* To provide professional advice, support and leadership to other Voluntary, Community, Faith and Social Enterprises (VCFSE) and statutory agencies and other members of the multidisciplinary team.
* To work with and monitor key performance indicators ensuring data quality is maintained.
* Monitor quality standards relating to service user and carer experience, including the monitoring, and reviewing in line with current clinical guidance practice and policy.
* Work with the team to ensure quality outcome reviews of the service users are completed
* To participate in multidisciplinary discussions and reviews about service users across the SIAS Partnership
* To support the team manager and all staff within SIAS with clinical leadership and clinical expertise and offer effective, professional clinical supervision within the teams.
* To line manage a number of staff.
* Support the Team Manager with ongoing service developments, improvements and quality standards.
* To deputise for Team Manager in their absence.
* To undertake non-medical nurse prescribing,
o Will act as an independent practitioner within the community settings, while liaising with other professionals as necessary.
o Will manage a caseload.
o Will be responsible for the assessment, planning, implementation and evaluation of care within agreed parameters, and have a working knowledge of local, national policies and legislation, which govern current service provision and audit.
* Practice as a non-medical independent nurse prescriber.
* Will provide professional clinical supervision to staff within SIAS.
* Provide expert support to nurse colleagues to provide outcome focussed interventions and
* Complete needs assessment and care plans for service users in both RiO and Illy
* Complete risk assessment and risk management documentation.
* Complete crisis planning and relapse prevention documentation.
* Assess and respond to carers' needs.
* Transfer of care and discharge planning.
* Will be responsible for ensuring effective communication pathways exist.
* Will be responsible for ensuring the promotion of equality, diversity and rights in accordance with good practice and legislation.
* Will be responsible for identifying and contributing to the training needs necessary in providing quality evidence based care as appropriate.

As part of the selection process you may be required to undergo a Disclosure and Barring Service (DBS) check, however all cases of criminal record history are dealt with on a case by case basis. Please note, if applicable for your role you will be required to cover the cost of your DBS certificate. This will be deducted from your salary over 3 months. You will also be required to sign up to the DBS Update Service before commencement with the Trust. The Trust will reimburse this. You will be contractually obliged to maintain your subscription.

All successful applicants will be required to complete the Trust's induction programme. The Fundamental (Statutory and Mandatory) training will be in an e-learning format. You will be required to complete training modules online (approx. 9-14 hours depending on post) by the end of your second week with us.

Relocation expenses may be reimbursed subject to eligibility. The policy provides for reimbursement up to £8,000.
