Main Duties
Working as part of the Nursing Service at Tynedale Hospice at Home, the Registered Nurse Care Coordinator plays a key role within the care coordination team, supporting the delivery of high‑quality, person‑centred care. The role involves triaging referrals, coordinating care and managing a caseload, ensuring patients receive timely and appropriate support. This includes allocating care to Registered Nurses, Healthcare Support Workers and an Apprentice Senior Healthcare Support Worker, based on individual patient needs.
This varied role combines office‑based coordination with clinical practice, including patient assessment and review, and effective use of digital systems to support safe and responsive care delivery. The post holder will also contribute to the development and support of junior staff, including mentoring responsibilities, and participate in the THH Traceability rota. Working closely with multidisciplinary teams across West Northumberland, the post holder will support integrated working, helping to improve access to high‑quality palliative and end‑of‑life care, ensuring patients and families receive compassionate, coordinated support when they need it most.
Essential Qualifications and Skills
* First Level Registered Nurse
* Minimum of 3 years post registration experience
* Proven ability to work both independently and collaboratively as part of a team
* Awareness of palliative and end‑of‑life care
* Excellent interpersonal skills
* Ability to interface effectively with a range of people in complex situations
Requirements
You must hold a full driving licence and have access to a car insured for business use. All posts are subject to a six‑month probationary period. Successful applicants are required to provide an enhanced disclosure with a check of the adult barred list; disclosure expense will be met by Tynedale Hospice at Home. Please note that we do not have a sponsor licence, and are therefore only able to appoint candidates who are eligible to work in the UK.
