Practice Leader Specialist Support (Mental Health)- Supported Living

In this role the successful candidate will provide practice oversight, coaching, training and line management to 3 teams that support 3 individuals in their own purpose built, brand new flats in the Hillsborough area of Sheffield. Together we will be providing exceptional person-centered support that promotes independence, mental health well-being, and community involvement to ensure the people we support lead the life they choose.
Upon completion of Induction Training and Probationary period the successful individual will have duty manager responsibilities, on a rota basis, within the leadership team.
About You:
* Minimum of 1 year experience in a similar role
* Experience in supporting individuals with mental health, including crisis support
* You can evidence improving outcomes for individuals with Mental Health needs
* Experience of supporting individuals with Autism.
* Experience of trauma informed care and utilising appropriate approaches.
* Experience of liaising with other professionals in a collaborative manner.
* Excellent spoken and written communication skills
* Line Management experience preferrable
Desirable
* A qualification demonstrating your experience, training and knowledge in Mental Health
* A full UK driving licence

Key Duties
* Reporting directly to the Registered Manager
* Use your experience to ensure the right support at the right time in collaboration with the Head of Specialist Support
* Provide practice oversight to your teams as well as on the job coaching
* Conduct Incident De Briefs in a timely manner, sharing lessons learned
* Conduct Care Needs Assessments linking in with local multi disciplinary teams
* Manage and coordinate digital care planning across your teams
* Ensure all staff are up to date with face to face and e-learning requirements
* Work closely with the people we support, their families and wider MDT
* Oversee all daily tasks for compliance including policies and procedure
* Support the Registered Manager to work towards an Outstanding CQC rating
The successful applicant will need to pass an enhanced DBS check and satisfactory referencing prior to commencing employment.
