Role Summary
The post holder will be responsible for the assessment of individuals in the community, home, nursing homes and hospitals to determine eligibility to receive NHS Continuing Healthcare or Funding Nursing Care.
The assessor will act as a member of the multidisciplinary team to co-ordinate the process, establish eligibility and assist in identifying a suitable care package. The assessor will ensure that the review process for all NHS Funded Care is undertaken in accordance with current guidance.
Key Responsibilities
* Utilise the national assessment tools and adhere to the required timescales.
* Co-ordinate, participate and support the assessment process with the multidisciplinary team.
* Determine individuals’ eligibility in line with the NHS Framework for NHS Continuing Healthcare and Funded Nursing Care.
* Explain the process to patients, families, health and social care colleagues and care providers, ensuring transparency and clarity for all parties.
