Job Summary
An exciting opportunity has arisen for a proactive and enthusiastic GP to work with the developing Hospital at Home (H@H) and Urgent and Unscheduled Care (U&UsC) Community teams based at Western Isles Hospital and supporting in‑reach to the Acute Assessment Unit (AAU) to facilitate direct admission to H@H or care at home. The role provides clinical leadership, a Senior Clinical Decision Maker position, and clinical input within the hospital and community. It supports training for the wider team, promotes a culture of admission avoidance and early discharge, and provides remote support to similar services at Uist & Barra Hospital and St Brendan’s Hospital on Barra.
Responsibilities
* Provide clinical leadership and act as a Senior Clinical Decision Maker (SCDM).
* Support and train the team of healthcare professionals in the hospital and community setting.
* Facilitate admission avoidance and early discharge with patients within the virtual H@H ward.
* Provide remote support to similar services at Uist & Barra Hospital and St Brendan’s Hospital on Barra.
* Assess new presentations in the community, at the front door of the Acute Assessment Unit and within the hospital environment.
* Collaborate with senior medical and nursing staff to ensure safe and appropriate facilitated discharge.
Qualifications
* Full UK driving licence.
* Willingness to innovate and help patients receive the best possible care in their home environment.
Working Hours
The service operates 7 days a week: 08:00‑22:00 Mon‑Fri and 08:00‑24:00 Sat‑Sun (split shifts possible). The schedule is suitable for part‑time work or medical staff bank.
Additional Information
* The post does not attract moving expenses or a Golden Hello.
* The successful applicant will be required to register with the PVG Scheme (Protecting Vulnerable Groups Scheme).
* No relocation expenses are available.
