Job Overview

This is an exciting full‑time opportunity for both experienced Advanced Assessment Nurses to join a dynamic and innovative team providing holistic assessment and treatment in a variety of community and acute settings designed to reduce preventable hospital admission. The referral source will typically be from an Acute Trust or other Community Rehabilitation in‑patient units.


The aim of the service

* Providing the Hillingdon borough population with an alternative to hospital admission through rapid deployment of clinical response to patients in their own homes.
* Providing a same day or next day response to make initial assessment and form onward plan.

The service operates daily from 08:00 – 20:00 including weekends and bank holidays. Typical shifts run from 08:00 – 20:00 and 09:00 – 17:00. Successful post holders must be able to work across 7 days per week in a full‑time post. As well as working in the community, work might include working in the Acute Trust as part of the Community Discharge to Assess Team. Successful candidates may be expected to support any other service demands.

The team is comprised of nurses, physiotherapists, occupational therapists of varying experience and support staff (including Nursing Associate), with access to GPs, pharmacists, case managers, social services, dietitians as well as other specialist nursing and therapy services located within the borough.


Main duties of the job

Candidate should be experienced and have a sound knowledge of the community. Will need the ability and clinical skills to work autonomously but also work alongside other members of the multidisciplinary team.

Detailed Job Description And Main Responsibilities

This role is for a senior advanced nurse working within a multidisciplinary community team, supporting the safe and timely transition of patients from hospital to community care. The post holder provides clinical leadership, helps manage the team, and deputises for the clinical lead when required.

They act as an expert clinical resource, carrying out complex assessments, making high‑level decisions, and ensuring patients receive appropriate care, rehabilitation, and follow‑up. The role involves working both independently and collaboratively with healthcare professionals across hospital and community settings, including Emergency services, GPs and social care teams.

Key responsibilities include assessing, planning, implementing and evaluating patient care; managing a caseload; supporting safe discharge; and ensuring high standards of evidence‑based practice. The post holder also leads and supports team members, delegates tasks, contributes to service development, and helps coordinate team activities.

Strong communication is essential, including working closely with multidisciplinary teams, leading meetings, maintaining accurate records, and supporting patients and carers with information and education.

The role also involves leadership and management duties such as supervising staff, contributing to training and development, participating in audits and quality improvement, and helping shape service delivery.

Overall, this is a senior clinical and leadership role focused on delivering high‑quality, patient‑centred care in the community while supporting team performance, service improvement, and professional standards.


Person specification


Essential criteria

* Substantial post registration experience.
* Current registration with NMC Professional body
* Mentorship or teaching qualification equivalent to ENB 998 / workplace assessor
* Experience of working in a multi‑disciplinary setting/team
* Ability to plan own workload and manage time effectively with minimum supervision and achieve a balance between clinical and other aspects of role
* IT literate with ability to use technology and other digital solutions with patients and in the operation of the service

* Supervisory experiences
* Project Management Experience
* Experience of leading, planning and completing audit / research.
