Overview
We are expanding our clinical capacity and looking for a highly motivated and enthusiastic part-time Diabetic Practice Nurse to join our friendly, dedicated team at Fountain Medical Centre. It is essential you have a Diabetic Diploma. The ideal candidate should have excellent people, planning and organisation skills and be able to manage your own workload.
The successful candidate will work alongside a multidisciplinary team including GPs, Advanced Nurse Practitioners, Clinical Pharmacists and Nursing staff to provide treatment and care to patients. Support, supervision and mentoring will be provided. An attractive package is offered, commensurate with experience and qualifications, NHS Pension, allocated study leave, 27 days annual leave (pro rata) plus statutory bank holidays.
Responsibilities
* Chronic Health reviews and management
* Cytology
* Wound care/Dressing management
* Travel Clinic
* Routine immunisations/Childhood immunisations
* Contraception checks
* Follow agreed clinical protocols with referral to senior nurse or GPs as appropriate
* Must be a Registered General Nurse with full NMC registration
* Diabetic diploma required (and other diplomas essential)
* Diabetic Practice Nurse experience is required; other training will be supported
* Clinical duties include: chronic disease management, vaccination programmes, cervical cytology support, and opportunistic wound care
* Participate in immunisation programmes for adults and children and vaccinations for international travel
* Work within policies and guidelines to deliver evidence-based care
Qualifications
* Essential:
o Registered first level nurse
o Minimum 3 years post-registration experience
o At least 2 years recent primary and community nursing experience
o Relevant qualifications for nurse-led clinics
o Knowledge of management of patients with long-term conditions
o Clinical skills in immunisation and vaccination, cytology, sexual health, ear care and chronic disease nurse-led clinic
o Knowledge of PGD and related policies
o Knowledge of health promotion strategies and local/national policies
o Experience with quality initiatives such as clinical benchmarking
o Knowledge and experience of working in primary care in a GP setting
* Desirable:
o Independent/supplementary nurse prescribing qualification
o Mentor/teaching qualification
o Clinical supervision training and experience
o Knowledge of public health issues
o Community nursing specialist qualification or equivalent
o Relevant nursing/health degree

Tuesday 12:00-16:00, Wednesday 08:00-18:00, Thursday 08:00-18:00, Friday 08:00-18:00
