Are you an enthusiastic pharmacist looking to move into or develop your existing career within primary care? The pharmacists will develop, enable, and support a culture of multidisciplinary care with a focus on developing PCN services in line with national and local specifications. Working directly with various practice teams, this role will link to colleagues in community and acute settings to ensure patient health and outcomes are met in a collaborative and innovative environment. The Pharmacist will support evening and weekend extended access clinics on a rota with the other PCN staff. Our current setup includes one remote evening per week and one on-site Saturday morning clinic every six weeks. The role requires a commitment to register on the CPPE pathway and Independent Prescribers Course (if these have not previously been completed); we will fully support you by providing time to attend training sessions, plus a GP clinical supervisor and peer support. You will collaborate closely with GPs and other healthcare professionals, conducting patient consultations both via telephone and face-to-face, assisting with care plans for long-term condition management (including hypertension, cardiovascular disease, asthma, diabetes and more), updating records, ensuring patient safety, and managing patients within the appropriate care setting, plus undertaking audits to ensure medication safety.

* Due to the geographical location, it would be beneficial to have a full driving license as travel will be required to practices across the PCN area.
* Join West Kent Primary Care and become a vital member of our Tunbridge Wells Primary Care Network team as a PCN Clinical Pharmacist. We are looking to expand our existing well-established clinic pharmacy team of 8 clinical pharmacists and 5 pharmacy technicians by recruiting a further clinical pharmacist who is passionate about developing their skills in General Practice.

Our PCN team, as a collective, is proactive and innovative with services delivered by not only our large clinical pharmacy team but also by first contact paramedics, first contact physiotherapists, Mental Health Navigators, Care Co-ordinators, a dedicated pharmacy team for our local care homes and more.
