Our successful client is seeking a passionate individual to join their team as a Care Coordinator, delivering exceptional home care services. The company is expanding across Home Care, Live-In Care, Day Care Social Activities, and Respite services. If you have experience as a Home Care/Domiciliary Care Coordinator, Scheduler, Senior Carer, Field Care Supervisor, or Care Team Leader, this is an excellent opportunity to advance your career.
Reporting to: Registered Care Manager
Key duties include:
1. Care Coordination, Rostering, and Scheduling for vulnerable clients in their homes, ensuring suitable staffing and service delivery.
2. Providing a safe, compliant, and high-quality home care service following CQC guidelines.
3. Prioritizing wellbeing for both staff and clients when scheduling care calls.
4. Developing care plans, conducting spot checks, and ensuring auditing and compliance.
5. Safeguarding clients and staff, conducting risk assessments.
6. Liaising with social care professionals and community partners, including social workers, GPs, NHS teams, and hospices.
7. Delivering personal care when necessary, participating in on-call rotations.
8. Promoting day care, live-in, and respite services locally.
9. Supervising care staff and team leaders, supporting their training and development.
10. Handling client and recruitment inquiries in collaboration with the care management team.
11. Staying updated on legal, safeguarding, and compliance requirements.
Desired experience:
* Proven experience in care coordination, rostering, and management within a home care setting.
* Experience liaising with clients, families, and healthcare professionals to ensure safe and compliant care.
* High standards of home care delivery.
* Team leadership and supervisory experience.
* Strong relationship-building and negotiation skills.
* Administrative competence.
* Knowledge of CQC regulations and safeguarding measures.
Desired qualifications: NVQ Level 2 and Level 3 in Health & Social Care.
