We are recruiting for a client for a Home Care Coordinator to join their service. The successful applicant will be based at the head office and will work in partnership with the current Home Care Coordinator's to lead the community care team and build on our current services.
The successful applicant must have the requirements below:
be able to drive & have a car, suitably insured for work purposes
Hold minimum Level 3 NVQ/Diploma in Health & Social Care (Adults)
- Bilingual skills are advantageous
Have at least 2 years continuous experience in a supervisory role within the domiciliary care industry
Be flexible to meet the varying needs of the business e.g will be required to participate in the out of hours on call service
The role can include the following:
* Coordinate care staff rotas and schedules in line with client needs
* Ensure care plans are followed and updated accurately
* Manage day-to-day issues with staff and service users
* Support and supervise care workers, including spot checks and shadowing
* Be part of the out-of-hours on-call rota
* Build and maintain strong relationships with service users, families, and local authorities
* Assist in audits, risk assessments, and compliance tasks
