Macmillan Colorectal / Stoma Care Support Worker

Job overview
The purpose of this role is to support the Colorectal and Stoma Care Specialist team in the delivery of nursing care and service to the patients who are on the Colorectal Cancer Pathway from pre-diagnosis to end of life care in secondary care and at any point of their colorectal pathway. The post holder will also be expected to support the inpatient stoma care service for patients who have had a stoma fashioned for either benign or malignant conditions. The post holder will be expected to work both autonomously or as a team member.
Main duties of the job
The successful candidate will provide support to the Colorectal and Stoma care nursing team and the patients that we care for by contributing to patient assessment and independently creating and also reviewing care plans .To provide support with clinical administration, including expediting results, distributing, and then completing being able to carry out electronic Holistic Needs Assessments (eHNA’s) with patients after the relevant training and support is given) either over the telephone or face to face in the ward or clinic environment.
· The post holder will also by expected to work with the Acute Stoma care team to provide inpatient stoma care to patients that have a colostomy, ileostomy, urostomy or nephrostomy as part of their treatment be this for either a benign or malignant condition. This includes provision of care at any site within the Northeast Sector of the Northern Care Alliance. You would be expected to review, provide stoma care education and support to patients with either an established or new ostomy.
· You will also be expected to participate in audit, contribute / produce both monthly and annual reports as well as feedbacking back data at the nursing and colorectal division meetings.

* NVQ 2/3 in Health Care
* Recognised Stoma Care qualification NVQ 2 or equivalent
