Overview
LPFT and South Lincolnshire Rural Primary Care Network are recruiting a Senior Mental Health Practitioner (registered mental health nurse, occupational therapist or social worker). The post will be based at The Munro Medical Centre, Spalding.
Responsibilities
* Work as an autonomous practitioner within specified GP practices to provide specialist mental health advice, interventions and training.
* Provide specialist clinical knowledge and interventions to service users in a primary care setting, supporting people with highly complex mental health needs/serious mental illness to achieve good physical health and wellbeing.
* Plan, implement, monitor, and review therapeutic interventions with individuals / carers who have serious mental illness through holistic patient-centred assessments.
* Provide best practice training, mentorship and guidance to mental health practitioners and other members of the primary care / neighbourhood MDT.
* Manage a caseload of highly complex cases, working with locality secondary mental health services to maintain personalised care plans focusing on what matters to the person.
* Attend core neighbourhood working meetings as part of an integrated team, working with the Neighbourhood Lead.
* Facilitate, participate in, and optimise case conferences / MDT meetings.
Requirements / Qualifications
* Experience as a clinician with proven skills in working with people who have highly complex mental health needs.
* Experience in evidence-based and innovative practice.
* Registered professional in nursing, occupational therapy, or social work (as applicable).
