Job summary

The post holder will independently assess, diagnose and manage patients withacute, chronic and complex medical conditions, coordinating care with other primary and secondary healthcare services as appropriate.

There is an expectation that the post holder will attend to patients within the practice and in their own homes and will coordinate care with other members of the organisation team and external providers.

Where the clinician is required to undertake home visits for housebound patients requiring acute medical assessment following triage by the duty GP, they will be expected to complete a maximum of 10 visits between 10:00am and 6:00pm on designated days. Each visit will be allocated 45 minutes.

The post holder will manage a clinical workload of 26 patients per working day with 15-minute appointments allocated per patient, when on site.

Where the post holder is not an independent prescriber, they will make recommendations regarding drug treatment and medication reviews, working collaboratively with a prescribing clinician.

Appropriate training, supervision and mentoring will be provided as necessary.


Main duties of the job

The post holder will provide first-contact care for patients presenting with undifferentiated acute and chronic conditions, undertaking clinical assessment, diagnosis, and management planning. They will develop and deliver patient-centred care plans, interpret diagnostic tests, and implement appropriate treatment, including health promotion and self-management support.

The role includes conducting home visits, and working collaboratively within the multidisciplinary team and external services to ensure coordinated care. The post holder will maintain accurate clinical records, participate in clinical governance, audit, and service improvement activities, and support enhanced services and organisational priorities.

They may also contribute to teaching, mentoring, and quality initiatives such as QOF and other incentive schemes, while engaging in ongoing professional development.

* Registered with appropriate professional body: NMC (nurses), HCPC (paramedics, physios, OTs), GPhC (Pharmacists), or GMC (PAs).
* Meets revalidation requirements in accordance with relevant professional body guidance (e.g., NMC, HCPC, GPhC ect).

* Qualified Independent Prescriber.
* ALS and PALS.
* Minor Illness Qualification.
