About the Role
You will serve as Team Leader at Greensand House Care Home in Surrey, supporting the home management team in delivering high‑quality care with a safe, caring, innovative, supportive, and inclusive environment for residents, visitors, and staff.
Other Responsibilities
* Lead the person‑centred care planning process, implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and ensuring completion of action plans.
* Assist with smooth transition of discharges, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate.
* Monitor the well‑being of each resident, identify any changes impacting care and care planning, and update records accordingly.
* Practice safe systems of work across a range of tasks, including moving and handling residents, assessing risks and ensuring safety for residents, visitors, and staff.
About You

Ideal Candidate
* NVQ 3 or equivalent in care for care homes.
* Two years’ experience in a senior position with residents.
* Effective communication skills, both verbal and written.
* Demonstrated ability to lead staff and act responsibly in the absence of the manager.

* Proof of eligibility to work in the UK.
