As a member of this busy but friendly team you will experience many core aspects of pharmacy practice, including clinical work, commitments to the dispensary and aseptics, and contribute to our 7-day working and on-call service. Pharmacists are part of an engaged and progressive Pharmacy team supporting hospital areas with staff coping with the increasing population and complex needs. Working closely with the multidisciplinary team, key aspects of this clinical post include attending multidisciplinary team meetings, patient education and counselling, clinical audit, medicines safety and medicines information. Opportunities for personal growth and development are available, including support for post-graduate qualifications in pharmacy, independent prescribing, and the opportunity to practice as a non-medical prescriber. The rotation includes experience across 2 dispensaries on 4 main sites and a full range of clinical activities.
Responsibilities and Activities
* Medicine: medical admissions unit, cardiology and gastroenterology.
* Surgery: ICU and Surgical Admissions.
* Orthopaedics: Wrightington Hospital; experience of anti-TNF therapy and Rheumatology with involvement from consultants & nurse specialists.
* Oncology and Parenteral Nutrition.
* Paediatrics, Women’s Health, Medicines Information, Medicines Safety, Clinical trials.
* Clinical Pharmacy Services to patients as part of the Clinical Pharmacy Team.
* Dispensary commitments and ensuring high professional standards therein.
* Rotation to experience major clinical areas within the Trust and to support 7-day services.
Department and Systems
The department is fully modernised and uses ARX robot, EMIS software, and has implemented Allscripts Electronic Prescribing in the last 3 years.
Key Results and Expectations
* Provide Specialist Clinical Pharmacy Services to patients as part of the Clinical Pharmacy Team.
* Deliver high standards in dispensary work; take professional responsibility in the absence of a more senior Pharmacist.
* Complete rotational training to experience all major clinical areas within the Trust.
* Ensure compliance with Medicines Law, CQC requirements and internal audits; comply with medicines management guidance from the Trust, GMMMG or national bodies.
* Complete Clinical Pharmacy Diploma.
Planning and Organisational Duties
* Prioritise patients by clinical and pharmaceutical need; obtain and validate medication histories.
* Advise clinicians on required changes to medication using clinical data and patient notes; analyse medicine charts considering clinical context.
* Endorsements in line with Pharmacy standards; ensure VTE compliance and antimicrobial prescribing per guidance.
* Coordinate with Medicines Management Technicians to order patient-specific medicines; promote reuse of patients’ own drugs where appropriate.
* Dispose of unusable medicines following procedures; dispense and hand out prescriptions per SOPs.
* Communicate medicine information to patients and healthcare staff to improve safety and effectiveness.
* Assist in cost-efficiency efforts by aligning medicine use with formularies or guidance.
* Manage pharmacy stock and assist with ordering, issuing and receiving medications as needed.
* Support active risk management and participate in clinical trials with proper documentation.
* Complete ward Controlled Drugs audits as scheduled; engage in developments, project work, audits and surveys as directed.
* Provide services to Oncology when suitably trained.
Communications and Key Working Relationships
* Assist other health professionals on ward to deliver high standards of care; support advanced Pharmacists in delivering Specialist Clinical Pharmacy Services.
* Liaise with pharmacy team to ensure clinically effective services; refer to more senior pharmacists when appropriate.
* Provide ward cover for other clinical teams or more senior colleagues as needed.
Finance, HR and Health & Safety
* Handle cash, prescription charges and issue receipts; sign orders for pharmaceutical products including controlled drugs in the absence of a more senior Pharmacist.
* Organise own work patterns within the framework provided by line management; supervise Band 6 pharmacists, pre-registration pharmacists, student pharmacy technicians, Medicines Management Technicians and assistants; promote understanding and implementation of procedures.
* Suggest improvements to procedures through line management.
* Comply with Health & Safety at Work Act; manage risk, conduct risk assessments, report incidents and use Datix to record medicines-related incidents; communicate known risks to line managers.
Teaching
Provide teaching to healthcare professionals, including sessions for Band 6 and pre-registration pharmacists, aligned with GPC expectations and tutor guidance.
