Job Summary
Band 6 (Crisis Resolution and Home Treatment Teams) posts are available in Derby City Team. Are you a dynamic, confident and motivated Mental Health Nurse/Occupational Therapist or Social Worker with excellent communication skills and the ability to assess and lead on clinical decision‑making? If so we would love to hear from you. This is an exciting role working in a fast‑paced environment within a well‑established, multi‑disciplinary team. The crisis and home treatment teams offer a high‑quality assessment and home‑treatment service as an alternative to hospital admission for individuals experiencing acute mental health crisis.
Key Responsibilities
* Provide first‑line assessments of people experiencing acute mental health difficulties and act as gatekeepers to acute admissions.
* Rapidly assess people with mental health problems who are vulnerable to hospital admission.
* Provide specialist crisis assessments with a focus on "hospital at home".
* Co‑ordinate the Crisis and Home Treatment Team on a shift‑by‑shift basis, acting as Duty Worker to triage referrals, plan shift activities and team accordingly.
* Participate in team MDMs.

Qualifications and Experience
* Qualified Mental Health Nurse, Occupational Therapist or Social Worker
* Current registration with the relevant professional body (e.g. NMC, HCPC, Social Work England)
* Previous and relevant post‑registration experience
* Ability to meet the travel requirements of the post
