We are seeking enthusiastic, compassionate Social Workers who hold a strong belief in recovery for people with severe and enduring mental health problems. Our Community Mental Health Social Workers hold a caseload and undertake the role of care coordinator for those with varied complex mental health and social care needs. They provide assessment and specific interventions to meet the needs of service users, their carers and families, and coordinate care programmes within a framework of recovery.
Responsibilities
 * Provide assessment, care planning and care coordination, including the Care Programme Approach, for adults experiencing or recovering from personality, substance abuse, and mental health difficulties.
 * Co‑ordinate care for complex and varied needs, ensuring carers are effectively assessed and supported.
 * Work as a member of a multi‑disciplinary team to deliver high quality social care in the community.
 * Demonstrate ability to work well within a team, prioritise and manage workload, and show initiative while maintaining a positive, professional and calm approach.
Qualifications and experience
 * Post‑graduate qualified Social Worker (social work or related field).
 * Experience in community mental health settings preferred.
 * Good understanding of the Care Programme Approach, Care Management and safeguarding.
 * Familiarity with the ASYE programme (for newly qualified Social Workers).
Working hours and location
Monday–Friday, 9 am–5 pm at our Cheshunt office, providing services to East & South East Hertfordshire.
