An exciting opportunity has arisen within Ipswich West Community Team. We are looking for an energetic Band 5 Physiotherapist who is passionate, committed, innovative, enthusiastic and highly motivated to deliver high quality care within patient’s homes.
You will be committed to teamwork, delivering high quality, holistic patient care, have a desire for ongoing professional development and ambition to mould and develop the services at a local level.
The therapy team are co-located within Constantine House Ipswich, and visits are scheduled geographically in the IP1, IP2, IP6, and IP8 postcode area. You will be working with a caseload including neurological conditions, falls assessment and management, and palliative care.
We have one permanent contract and one fixed term, both are full time 37.5 hours, the fixed term position is until 31/3/2026.
You will be working alongside and supported by other Occupational Therapists, Physiotherapists, Community and District Nurses, and Generic Support Workers.
1. To be an active member of a multi-disciplinary, integrated community team.
2. To manage a defined caseload of complex individuals, with the support of the Band 6 occupational therapists, Physiotherapists and clinical Lead using evidence based/person centred principles to assess, plan, implement & evaluate therapy interventions.
3. To provide clinical supervision and supervise band 3 & 4 staff.
We will provide support through in-house professional forums, Mandatory training, 1:1 and personal development meetings as well as clinical rolling skills training via the practice development facilitators. We facilitate students on training and have close working relationships with neighbouring teams in Health & Social Care and primary care.
Previous community experience is preferred; but training will be available to applicants who possess the personal qualities/skills required.
You will need to be able to meet the needs of this service by being available to work flexibly between the hours of 08.00–20.00 on a rota basis over a 7 day period.
