Palliative Medicine Pathway Coordinator (Administrator)

An exciting opportunity has arisen within the Palliative Care Team for an administrator. The post holder will provide an efficient, effective and friendly secretarial service to patients and professionals within the Hospital Palliative Care Team (HPCT) and Community Palliative Care Team (CPCT).
We are looking for an enthusiastic and motivated team player with excellent communication and problem‑solving skills. You will require high levels of tact and diplomacy and an ability to deal with queries from patients and professionals in a sensitive manner.
Main duties of the job
* Act as the point of telephone contact for patients and their relatives/carers who may be distressed and highly anxious, ensuring all queries are answered with sensitivity and in a courteous and professional manner; messages are passed onto relevant team members, and patients are signposted to the most appropriate place.
* Act as a point of contact for professionals, including responding to referrals and phone queries relating to patients on the caseload; ensure messages are directed to the most appropriate individual, and complex clinical concerns are escalated.
* Process routine and urgent referrals, ensuring all referrals are responded to in a timely manner.
* Diary management, including reviewing patient appointments and supporting scheduling support calls.
* Provide ad hoc administrative duties to support the service to run effectively and efficiently.
* Liaise effectively with wards and departments within the hospital, as well as external parties including GP and neighbouring Trusts.
* Assist with the ordering and maintenance of stock.

Qualifications
* Demonstratable numeracy and literacy skills.
* Well written personal statement.
Experience
* Customer or patient care experience.
* Knowledge of administrative procedures.
* Experience working in an administrative role.
Skills
* Communication skills and ability to give non‑clinical advice.
* Ability to use IT, including Microsoft.
* Ability to work to deadlines and prioritise workload.
* Experience having challenging conversations.
* Training in communications skills.
