Your 'supporter' role will be to encourage us to live as independently as possible, promoting inclusion within our local community and supporting us to be active citizens. You will follow direction from us, our families and the important people in our lives, you should be proactive and contribute to fulfilling our life goals and aspirations.
You need to be adaptable and flexible, supporting us weekdays, alternative weekends and some sleep-in shifts.
Thinking and acting in a truly person-centred way, you will be making a positive difference to our lives, promoting our health and well-being, following, contributing to, and evolving our support aims and outcomes as we grow.
We need you to be a good planner and timekeeper, supporting us to attend appointments and meet our support goals, actively participating in our support review sessions with social care professionals.
We ask you have a good knowledge of our local area, supporting us within our home and getting out and about to find life-enhancing social activities and voluntary or work opportunities.
You must have a driving license and your own vehicle to support us to attend our health appointments.
You will need to have qualifications in health and social care or work towards gaining them. Training will need to be completed monthly to stay competent and effective in safeguarding, risk assessing and understanding health needs such as cerebral palsy, epilepsy and upholding our human rights.

Taylor Made Care is looking to recruit a passionate support worker to join with our existing team. As a support worker you are responsible to promote a supportive and caring environment, as part of a team, providing support and emotional support to citizens, enabling them to achieve their desired level of self-sufficiency, promoting independence and support to access the community and any health appointments.
Main responsibilities-
Support tenants and enable them to carry out day to day personal tasks, which may include the taking of medication, developing personal relationships, personal care and any dietary needs.
Support colleagues by working with tenants in the implementation, monitoring and regular review of individual support plans.
Support with meal preparation and laundry if required by tenants support plans
Communicate with tenants on a day to day basis ensuring that all necessary services including medical and social services are provided as directed by the manager and in line with tenants wishes.
Liaise with outside agencies and partners on behalf of tenants
Support tenants in line with the agreed support plans to take part in external activities including access to leisure, education and employment opportunities.
Support tenants with finances where this agreed in their support plan involving assisting with the management of petty cash system and recording and maintaining accurate records. Support workers will signpost to access appropriate support to complete benefits claim.
