The Pharmacy Department at Lynfield Mount Hospital is seeking an experienced Medicines Optimisation Pharmacy Technician to join their busy team. The role involves providing medicines optimisation for Mental Health Services to enhance the quality of care for service users regarding their medicines at Bradford District Care NHS Foundation Trust.
Responsibilities:
1. Dispensing and issuing various prescriptions, including accuracy checking before supply to wards/teams and outpatients.
2. Collaborating with the team to supply stock and non-stock items to inpatient and off-site areas, including community clinics.
3. Visiting wards to facilitate medicines reconciliation for new admissions.
4. Assisting with clozapine-related activities to deliver high-quality service at Lynfield Mount Hospital and Meridian House.
5. Providing advice to service users and carers in inpatient and outpatient settings.
6. Supporting the Tobacco Dependency and Treatment service as a trained advisor.
