What's the role about?
As a Social Care Coordinator for Education, Health and Care Plans (EHCPs), you'll ensure that the social care elements of EHCPs are robust, timely, and aligned with national standards. You'll work with children and young people aged 0-25, including those already open to social work services, ensuring their care needs are accurately reflected. Your work will support the local authority in meeting its statutory responsibilities under the SEND Code of Practice.
Daily responsibilities include:
* Coordinating and completing social care elements of EHCPs
* Collaborating with families, social workers, and professionals
* Attending reviews and multi-agency meetings
* Ensuring the child's voice and family needs are prioritized
* Maintaining accurate records
* Staying informed on legislation
* Providing guidance and support to colleagues
Who are we looking for?
We seek someone with experience, passion, and professionalism. A background in social care or family support, understanding of SEND legislation and safeguarding, excellent communication skills, organisation, and adaptability are essential. A team-oriented attitude and commitment to high-quality support are also important.
