Offer description

Responsibilities

* Undertake the general duties of a Practice Nurse focusing on Chronic Disease
* Be competent to offer chronic disease management to support the practice in achieving quality indicators and targets of QOF, LIS and DES, including COPD, Asthma, Heart Failure and Diabetes
* Carry out childhood immunisations
* Offer travel health advice and vaccinations
* Perform general treatment room duties including wound management and minor injuries
* Provide family planning and contraceptive advice as required
* Provide appropriate treatment to the practice population in accordance with practice protocols and procedures
* Apply infection control measures within the practice according to local and national guidance
* Maintain accurate records and document consultations according to local and NMC guidelines
* Ensure awareness of statutory and local clinical protection procedures, including systems of referral such as child protection strategies
* Work in accordance with PGDs and support HCAs working to PSDs
* Liaise with other primary health care team members as required
* Record patient details and consultations accurately on EMIS
* Chaperone for GP appointments when requested
* Actively participate in nursing meetings, following up on actions as required
* Be responsible for standard operating procedures and policies within own area of work, ensuring they are kept up to date
* Liaise with team members regarding changes to service
* Ensure annual leave is covered with other staff members in advance and inform the Practice Manager of any staff shortages


Qualifications

* Hold a non-medical prescribing qualification, or willingness to undertake training to achieve this
* Qualified to undertake cytology screening

* Participate in continuing education as identified personally or by the Practice to maintain a high level of professional knowledge and skills
* Promote continuing professional development of yourself and other nurses and healthcare assistants, facilitating a learning culture within the team
* Offer innovative ways of working and opportunities to facilitate learning

* Communicate effectively with other team members, patients and carers
* Recognise people's needs for alternative methods of communication and respond accordingly


Work Setting & Review

* Work under own initiative, seeking advice either clinically or managerially as required
* Recognise work within own competence and other requirements as regulated by the NMC
* Prioritise duties to ensure patient needs are met
* Attend and complete mandatory training as directed by the practice
* Adhere to practice policies and procedures
* Participate in an annual individual performance review, including maintaining a record of own personal and/or professional development
* Identify personal development and training needs in conjunction with clinical lead and Practice Manager
* Take responsibility for own development, learning and performance and demonstrate skills and activities to other team members


Confidentiality

* Respect confidentiality of patient information and adhere to the practice's policies and procedures
* Access to confidential information is strictly confidential and may only be divulged to authorised persons in accordance with policies


Health & Safety

* Comply with all health and safety policies, including completing mandatory training
* Maintain up‑to‑date knowledge of health and safety, infection control guidelines and ensure implementation across the business
* Use personal security systems within the workplace according to practice guidelines
* Identify risks involved in work activities and conduct risk assessments as required
* Disciplinary procedures apply if supplies are misused recklessly or intentionally


Information Governance

* Keep up to date with information governance requirements and follow practice policies to handle information legally, securely and efficiently
* Appropriately manage records and make them available for sharing in a controlled manner, subject to statutory requirements and agreed security policies
* Ensure records are legible, attributable and contemporaneous
