About The Role
A unique opportunity has arisen for an Education Mental Health Practitioner (EMHP) to join the New Haringey Integrated Transitions Service to provide mental health support within educational settings to transform the way young people with emotional and mental health difficulties are supported to transition into adulthood. The role will focus on providing mental health support work with young people and their parents or carers to deliver evidence-based early interventions for mild to moderate mental health issues such as anxiety, depression, and behavioural problems and to use the acquired skills, knowledge and abilities to deliver a service that builds on and reinforces existing initiatives. The Educational Mental Health Practitioner (EMHP) will be a key member of the specialist multi-disciplinary team within the Haringey Integrated Transitions Service.
About The Team
The Haringey Integrated Transitions Service: Steps to Adulthood is a new multi-disciplinary initiative supporting children/young people with mild to moderate mental health needs as they transition into adulthood. It bridges the gap between child and adult services by providing early intervention, continuity of care, and tailored support.
Mental health professionals will join an established team of social workers to work collaboratively with children/young people, families, and schools, delivering evidence-based interventions and promoting emotional wellbeing.
This service is designed to empower young people, enhance support networks, and improve long-term outcomes through an integrated, strengths based approach.
About You
You are a dedicated educational mental health professional with experience delivering outcome-focused, evidence-based interventions in educational settings for children and young people facing mild to moderate mental health challenges. You work collaboratively with young people, their families, and educators to develop tailored intervention plans and empower informed decision-making. You are skilled in conducting Mental Capacity Assessments for those aged 16 and over, monitoring progress, and integrating support around transitions, education, training, and employment. Your approach includes thorough risk assessments and clear risk management practices aligned with local procedures.
